Blank settings page on multisite

Using version 1.0.4 of the plugin on a multisite install and the settings page comes up blank (at /wp-admin/admin.php?page=brightcove-sources).

No relevant console errors and no relevant errors in the PHP error log.
I’m seeing this behavior on both my local dev env and a remote testing site.

I’m not entirely sure how to go about troubleshooting this.
