/blog changed to /blog/category/blog

Hi, i dont know what happened, all of a sudden i cant access to mysite.com/blog. it shows “not found” page. Now my /blog is in /blog/category/blog.
What happened? How can i fix it?
Please, need help ??
I already tried changin permalinks with no luck
