Broken thumbnail image in media library

I wonder if anyone has encountered similar issue where the thumbnail pulled from Youtube appeared as broken image thumbnail in the media library. This caused the post with the thumbnail as the featured image to be unable to display correctly.

When I test it with the built in debugger, it was ok. Anyone could give me some hint on how I could resolve this?
