Bug: don’t calculate correctly in percentage mode

There is an error in function adv_cod_add_payment_gateway_extra_charges_row if extra_charges_type==”percentage”
The amount of charge is always the base charge, even if set in percent mode. Need to correct 2 or 3 lines of code.
