Bugfixes – block-all-mixed-content and report-uri

Hi,

I fixed a couple issues I found. Is there a public repository where I may submit a pull request?

1. The block-all-mixed-content directive had a value of “1”
   when enabled which caused a console error in Chrome. This value has been removed according to the spec.
2. The report-uri was hardcoded and only active in report-only mode. I’ve made the report-uri configurable through the UI and now it can be active in either report-only or enforce mode
