Button brings up "Page Not Found" in the box

This is a GREAT plugin – and I love that you can easily add a custom DIV and add background colors, etc.
I have 2 sites I use this on, both are WP version 3.9.2. When in visual editor, I click on the ezcolumns button and it brings up the screen properly.
However on my other site, pressing the button gives me a “File Not Found” page. The dialog box is the same size, but my column options don’t display at all.
Any ideas?
