Yes, this is possible. You can use the calculation fields to make the same. Also, you would need to enable calculations to fields you would add to the form. Here is a demo form I made for you which can be useful to understand how to make a form for your use.

Kindly copy the RAW Paste Data from the above link. Then navigate to your site WP Admin Dashbaord-> Forminator-> Forms-> Import and paste the data there and click the import button. The form would then be imported.
You can now copy the shortcode for that form by clicking the gear button beside the form name and paste it to any page. You can also edit the form by clicking the edit button.
