call to action doesnt work with short code or other

    HI,
    I love you app, but.. I have a call to action button, (using Responsive theme by cyberchimps) and the call to action button will not work with the short code.

    It will work if I put the full URL address of the form, but it will ONLY work on one computer. it will not work on my android phone, another computer, or an IPhone/

    it gives me the 404 error message in one case and a blank page on another.
