Can I add new fields to the front end form? And another question

First, I really enjoy this plugin, it’s functionality is exactly what I’m looking for. But can I add custom fields or change the labels on the front end form?
And when is the success or failure message displayed? Is that sent as an email to the user who fills out the form?
