Can I use Contact Form 7 in combination with MySQL data?

  • Is it possible to create a Contact Form 7 form that contains data from a MySQL table to get the same as this:

    Form with MySQL data

    The dates, hours and matches come from the MySQL table.

    Yes I can, this is my form in WordPress:

    But how can I control that all fields are filled in before submit?
    How can I keep the filled in values after submit?
