Can No Longer Attach Fields to Forms

I just upgraded to Custom Contact Forms Version 5.1.0.2 and I can no longer attach fields to my forms. Prior to the upgrade, it worked just fine. What can I do to fix this? I tried both IE and Firefox, and nothing happens after I click “attach”. Additionally, to delete the form, I have to use a bulk action – the delete button doesn’t work either.
