Can not download files

There are some errors shown in the browser console for this page, including a jQuery error for Forminator. Do you currently have any optimization plugin or theme optimization option enabled which may adjust or optimize assets from plugins? If so, please try to deactivate or exclude Forminator assets.

Another option is to enable Load form using Ajax from the Form settings > Behavior > Rendering option and recheck if form is still having issues.
