Can not upload logo's

Logo is not being uploaded. CHMOD is 775 and 777 as well on the plugin folder as the uploads folder.

Tested on WordPress 4.3.1 and within 2 different browsers (same hostingserver, different databases).

Any ideas what can be wrong?
