Can you make a multi-step form?

Say you have a Yes/NO radio button, once you select Yes or No the form outputs the next condition, say it’s a text input field.

From here you can either cancel the text field (with a cancel button) and return to the Y/N radio buttons or after entering text press a Yes button and continue to the next condition.

Is this possible or does the entire form have to be displayed all at once?

    Hi @homelesshenry,

Trust you are doing good, and thank you for reaching out to us.

This is possible with adding visibility rules, please find more about the same in our documentation here: https://wpmudev.com/docs/wpmu-dev-plugins/forminator/#Forminator-Conditional-Logic

From here you can either cancel the text field (with a cancel button) and return to the Y/N radio buttons or after entering text press a Yes button and continue to the next condition.
I am a bit confused about this workflow, can you please explain this a bit more? Is that you are expecting the form to navigate to a new section when ‘Yes’ is selected on the radio button? If yes, I am afraid this is not possible with Forminator out of the box.

    Kind Regards,
    Nebu John

    Thanks Nebu,

I had actually just figured it out prior to your message.

If I could ask something else, what I’m wondering is whether the form can log the page/post it was submitted on by the user?

My plan was to output the form via shortcode in either the sidebar or at end of post, and was hoping to be able to have the recorded response also show the page it was recorded from.

Essentially, I’m trying to capture feedback on each post I make, but I need to know which post/page the feedback was sent from.

Actually looks like I figured that out as well.

Great plugin, you guys had a lot of foresight.
