cannot insert images in posts or pages

  • In a fairly new install of WP for no apparent reason I can no longer insert images in posts or pages. All was working fine until a couple of days ago.

    I have tried all the usual tests, deactivating plugins, re-installing WP-content and WP-includes and I’m using the default Twenty Eleven theme.

    I am able to insert images manually using HTML code but when I use the graphical method I get a ‘Page not found’ error. This happens with uploaded images and with those from the media library.

    I have read several posts regarding this problem but have yet to find a solution.

    Precondition Failed

    The precondition on the request for the URL /wp-admin/media-upload.php evaluated to false.

    Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
