Cannot uncheck settings in 5.0

Ack! I’ve seen (and fixed) that issue in the past, but I forgot about it, and since I had to redo pretty much all the option saving code, I forgot to handle that condition. You see, HTML forms do not submit unchecked checkboxes, so kinda hard to save something that isn’t submitted. ??

I’ve added extra fields to the form, to recreate these missing checkbox values, so all should be good for the future now. ??
