Cannot upload files after updating

After updating from 1.5.9.4 to 2.1.6:

1. on one tab the option to upload a file disappears although the description text is visible. 2. on this same tab, another input field (type=number) disappears.
3. on another tab the email address input field disappears but the description text is visible.
