Can't find the right poll/survey plugin

  • My requirement is to create a poll/survey of 5 questions with questions 1 and 2 open-text/free-format answers; questions 3-5 selecting from a pre-defined list.

    While some plugins do one type and others the other, I’m really struggling to find a solution for both.

    Help would be gratefully received.
