Can't log in to site – page refreshes

  • I’ve only recently encountered a problem whereby neither myself or the other admins and users can log into the site.

    The action taken to repeat this problem is:
    1) enter your username in the login box
    2) enter your password in the login box
    3) click the login button

    Result:

    The pages refreshes, the username remains in the login box, but the password disappears. No error message is shown.

    Currently I can log in if I choose forgot password and then follow the steps there to create a new password. However after that session has expired, or on logout, I am unable to log in again.

    Has anyone experienced this?

    Does anyone have any idea as to the issue?
