Cant Unsubscribe

Anytime someone trys to un-subscribe from my blog they get a notification that says “successfully subscribed” …

I have the widget placed on a page within my site with the short code: [subscribe2]

This code shows both the subscribe and un-subscribe button.. but when you click the unsubscribe button it does not unsubscribe you…

What is the easiest way to make a simple unsubscribe button so if they enter their email address it will automatically unsubscribe them from all emails?
