captcha image showing 404 error

Hi team,

I have been using this extension from last many months. But from last few days I’m getting 404 error image instead of captcha image for first time.
I mean to say that whenever I open the form for first time, it displays 404 error message, but when I refresh the page again, it displays the correct image.

So can you please solve this issue or help me out, since it is causing problem on my live site.
