Capturing 'City' and 'Country' for donors

It tells you how to add basic fields to your donations forms and can be expanded upon to add city and country fields as you require. You can then display these fields in the admin, send them within emails, and even expose them to the Give API. It should be everything you need for the code-savvy developer.
