Categories and Custom Menus Disappearing

A short while ago all of the categories disappeared from my site.

When I go to Admin > Posts > Categories … there is no categories to be seen, it says there are no categories basically – but above it says “20 items”

I tried the search option above and it still doesn’t show up.

I tried re-adding categories but they just disappeared too.

Any ideas?
