Categories Gone After Upgrade

Has anyone solved the problem of after updating to the latest WP, they are no longer able to add categories, even though they remain in the wp_terms table, and are visible when editing exisiting posts – which remain uncategorized even after saving the edited post.

Also, is there a way to start with a clean install, and just import posts and categories…
