CF7 showing "default" form and not my form in sidebar

I made a custom form in CF7 and when I place the shortcode into a text widget in the sidebar, it displays a form but it’s not the one I created, rather a default form. When the shortcode is placed into a post, it works fine.

Also, don’t know if this has anything to do with it, but the “Settings” option in the WP menu for CF7 doesn’t show up, and the CF7 widget that used to be with it isn’t in the Widgets panel either.

I’m using the Sterling theme on WP 3.5.1
