change comment form

Good evening,
I hope somebody can help me on this english forum, in the dutch forum nothing is going on :-(.
This post looks like a long post, but I put in the code I have found in my FTP files because I did a lot of searching but don’t know what to do!
I would like to adjust my comment form:
– email must be visible;
– the field to fill in the URL can be removed;
– if possible I would like to have an extra field where people can give a name to the post (and this should be visible too).How can I achieve this?
