Change product image with selection

In general, product add-ons are loaded based upon the mapping to the Product or the Product variation. As a result, the selected option by the customer on the Product page for the variation will show the add-ons. Product add-ons will not change any Product images in WooCommerce. Hopefully that answered your question.
