What I can see right into you code you are using textarea to show the fields.

You should be using input type text in place of that

Thank you. Where is the data stored for this custom field? I have done a test donation but I do not see the custom field data anywhere. There is no “export” tab in my “Reports”.

It looks like you’re storing that data to the payment meta within myprefix123_give_donations_save_custom_fields(). To export you can use this plugin: https://github.com/WordImpress/Give-CSV-Toolbox/

As well, I just want to mention if you’re not comfortable with code I would recommend using Form Field Manager rather than custom code.

I would like to have the field manager plugin but I have a really small budget for my project so I can’t buy it right now ?? But I am learning a lot about code ?? I am very grateful for your help!!!

Export options are under Donations > Tools. Glad you’re learning some code!
