    You can adjust your uploader to this style using css.

    Yes, please and I’d like to add extra fields as the fields will show up on another page…I want to build a new page and be able to do the upload…

    – Name
    – Email
    – Phone
    – Comments

    That’s the URL, the plugin now is disabled, because I need to know how I can add the CSS to fix this, and enable it…

    What do you mean by “the fields will show up on another page”?

    If you want to add extra fields on the upload form, open the visual editor of the form (instructions), go to Additional Fields and add as many fields as you want from there (here are instructions for the fields).

    Please note that I cannot help you with css if the form is disabled. I need to see it on your page, because the css depends on the theme. You can add a new page and put the upload form there.
