Changing Comment Section

Hi,

I would like to change the comment section from the standard in the theme template similar to what the author has on his webpage. I’ve googled a bit and I understand that there are inputs to the comment_form() function. So my question is what should the input array be to get a comment form similar to that of https://mycyberuniverse.com/anarcho-notepad.html?
