changing link name on attachment page using a permalink

I have set up a section in my widget area to have some forms patients can download and print or fill out. I havre attached a pdef file to the widget area and when I attached the form it uased a perma link and now it just says ?attachment_id=446 here is a link to the page. I would like to have the form show up and than have the patient click it to fill out or print out. I have changed my settings in permalink settings to be custome but when I type in the title
i want i hit save and it defalts to the ?attachment 1d=446? How can I get it to show up the title I want ??
