Check box not appearing?

Hello!
I use this plugin on my other websites, and it’s fantastic. But I just installed it on my new website, and I have the settings configured so that the checkbox should appear unchecked. But it doesn’t appear. The text does, but the text box doesn’t. I tried uninstalling and then reinstalling the plugin, but that didn’t work. Help?
