Check Boxes Not Working – unable to enter UA code or connect with Google

I have installed this plugin before and while I have never been able to connect directly with google, I have always been able to enter the UA code. This time, not only am I unable to connect with Google, but none of the check boxes function. I have tried deleting the plug in (re-downloading a new copy) and reinstalling it, disabling all plug-ins and only trying this one and nothing seems to be working. I have two other wordpress sites on the same server and they are both working fine (php 5). I am using ‘Platform’ as my theme, but I have also tried using the basic wordpress themes with no positive result. I have seen other posters have had trouble with the check boxes not functioning – any ideas on what I should be looking for to resolve this?
