Check form fields

Hi, I need help. I have to build a form where there’re five check fields. I need to exclude, when the customer checks one, the others. For example:

1: paint
2: photo
3: video

If it’s selected paint the customers can’t check the last two fields.

It’s possible? Some ideas?
