Some themes override the booking form in my plugin. That may be the case for you. Try switching to a default theme like TwentySeventeen to see if the field appears.

Perhaps you’re having issues with the cache in your browser? Try opening it in a different browser to see if it appears.

I had a quick look at the styles applied to that <input> field, but I didn’t see anything that looked bad. I’m afraid you’ll need to talk to your theme developer for help debugging this issue.
