Checkbox does not react do editing

Hello,

i have created form on page, all was good and worked properly. After few days I tried to edit form and options from checkboxes disapeared. From about 10 options remained only 3. And now it is unable to add them back. In admin area I will add them, but after save, they will not appear on page. After reload of admin area, just three will stay and no more. This happens all the time.
I tried to create another form, same problem
