Checkbox list incorrect layout

Please tell me what contact form template you are using. Also please provide link to the page with contact form. We will help you to solve the problem.

I use the simple white template.

Please also note the alignmen of the values

We will fix the issue with label of checkbox list and will release fixes in next version of contact form plugin on Monday.

As for the issue with alignment of values – it is connected with styles of your theme – https://wordpress-dev.advantive.nl/wordpress/wp-content/themes/Advantive/style.css – in this file the width is specified as 100% for all tables – https://prntscr.com/d8ghjo
