Checkbox: Send me a copy to my Email

Hi,

When a user fills out a form he must be able to send a copy of the form to his own E-mail (same as the administrtor receives).

This should be done with a Checkbox.

a) If he checks the checkbox, then he will receive a copy of the form he filled out in his E-mail account.
b) If he doesn’t check the checkbox then he doesn’t receive a copy of the form in his E-mail account.

How is this possible to achieve?

P.S. we don’t want the form to be sent generically to his E-mail account – the client (user) must be able to check a box.
