Checkbox to Agree to Terms

Hi.. on some of my download options, I’d like to add a “check this box to agree” which will force the user to acknowledge the terms and conditions of downloading the report. Ideally this would be a pop-up box. But a checkbox below the download button would also work.

Appreciate any advice on best way to implement with Download Monitor. Would your Gravity Forms extension give this capability?
