Checkout and Cart Not Matching – Cart Adding Random Items for Customers

Last night a client of mine started having customers report that their cart and the items in checkout were not matching. The problem is not all customers are having this problem. Some are and some are not, which is really strange and making it tough to pinpoint the issue.
I’ve been able to replicate it the issue and here’s what’s happening.
1) Customer adds a product to their cart.
2) They click on “view cart” and suddenly a whole bunch of other products will show up in their cart, but the cart icon will show the right amount of items and the right total.
3) I’ve found if the customer click the X next to one of the items that just started appearing the cart will go back to normal showing only the product the customer chose.
4) However, when the customer clicks checkout, it’s not always showing the right products in Checkout.
I thought it may be a caching thing and there was a caching plugin that was enabled, so I deactivated it. I’ve cleared my browser cache and cookies and instructed the customer to do so as well. However, after all that the issue still persists. There were not any plugins updated or changes made to the site immediately before this started happening, so I’m at a loss at what could be triggering this.
