City and state dropdowns not loading

Hi,
I am new to wordpress.
I am using this plugin for my contact form.
When I select option in country it is not loading state and city list at all.
Can you please help me with this?

Also on more thing does this plugin have feature where I can show up checkbox options depending on radio buttons selected respectively?

Let me know what should I do?
