Collection stops at 6 signatories

Hi there,

The plugin isn’t collecting beyond 6 signatories for my site. The form responds with the thank you message, indicating that the signatory should have been successfully collected. However, when refreshing the page the name hasn’t been added to the signatory list and when checking the back-end, the name hasn’t been added to the list.
