If you edited the plugin files directly and installed a plugin update, WordPress has overwritten your modifications.

WordPress removes the plugin and theme files and replaces them with the latest versions.

You can create a custom template and use with multiple forms by following the process described in the following blog post:

An alternative would be re-define the styles directly on your form’s structure by entering the new styles into the “Customize Form Design” attribute in the “Form Setts” tab:
