Comment Approval Not Working

We have been using the Facebook comments plugin with no problem for about 6 months. I have the settings set so comments must be approved before they appear on a post.

All the sudden, two weeks ago, comments starting appearing without first being approved. The moderation tab has also disappeared from our site.

I checked compatibility with other plugins and didn’t find any issues there. I also updated this plugin–after I was having issues and that did not help.
