Comment form has changed

  • I have version 2.1.3 on my blog https://momcooks.net (I’ll upgrade today, I promise!). A friend said she had a problem leaving a comment, so I went into Options–>discussion and unchecked the box for commenters have to leave their name and email. But then I changed my mind, so I went in and checked the box again. But now, all I see in the comment field is a place for a name and then the message. I’ve lost the spaces for the email address and website URL. How can I get them back?
