Comment form on separate page?

Hello,

I’m looking for a way to move the comment form on the bottom of each post onto a separate page like the comments system on A List Apart.

E.g. the post is here: https://www.alistapart.com/articles/vexing-viewports/
But the comments page is here: https://www.alistapart.com/comments/vexing-viewports/

Anyone know a way to implement something similar?
