Seriously, WordPress is simply too big and too complex to provide everything you are asking for in a single document.

You can browse directory structure and source code directly from trac fairly easily. The contents of the files themselves are usually very descriptive. A nice jumping off point for specific key word searches in the Codex. ie… header.php or sidebar.php
