Complicated Contact Form

  • I need to make a contact form that looks exactly like the pdf form in the URL link. Clients must be able to fill in the form online.
    Is there an existing plugin that I can use/buy

    ATM I am using Ultimate Form Builder Lite. The problem is that I can not make that many columns.

    All suggestions are welcome
