Conditional Fields

I am sure this is possible and really easy but I need a little guidance. I am using the pro version. Made a standard employment application. I need some conditional fields to pop up.

For example one of the questions on the application is a yes/no radial button. “Have you ever worked under another name? yes no” If they pick yes, I need to have a text field pop up that says “what name: ___________”
