Conditional fields are not working in Contact Form 7 Plugin

Hi,
I am using “Conditional fields for Contact Form 7” plugin to rendering fields based on conditions but unfortunately, fields are rendered based conditions.URL: https://finitegroup.com.au/engage-us-2/
Can anybody help me on this !!It is very urgent.
